The Assisted Living Waiver Program is designed to enable low-income, Medi-Cal eligible seniors and persons with disabilities remain in their community as an alternative to residing in a skilled nursing facility.
Also, to coordinate a transfer from skilled nursing facility to an Assisted Living Facility or a Board & Care setting.
This is a per-diem position covering the Inland Empire area.
Responsibilities -Assist in monitoring incoming requests made for the Assisted Living Waiver program.
-Collect data, important health information, insurance information, and medical histories from interested clients.
-Follow all eligibility protocols for incoming requests.
-Gather information about the client and create client records.
-Create consent forms to send to designated party and obtain signatures.
- Collaborate with a team of nurses and stakeholders to schedule assessments in an organized fashion.
-Gather necessary paperwork for submission of client's applications.
-Answer questions related to services offered through the ALW program.
Requirements -A bachelors degree is Social work is preferred, but bachelors in sociology and/or psychology will also be considered - Case Manager experience preferred -Must have excellent communication and organizational skills -Experience working with elderly populations and developing community relationships.
