Responsibilities:
Assess the participants to determine the needs, wishes, and desires for appropriate care for the participants attending to ADHC/CBAS center by developing and implementing Individual Care Plan virtually during the COVID-19.
Working as a member of the interdisciplinary team and focus on person-centered care practice.
Conducting the wellness check and risk assessment to the participants might provide in-home and in-center services base on the needs of the participants.
Act as a liaison with participants Primary Care Providers.
Report to MDT and Program Director on a regular basis or as needed.
Provide professional in-service training to the staff members and education to the participants.
Qualifications:
RN licensed by the State of California Bilingual:
English/Spanish preferred Be a team member and have the understanding and passion of older adults and families.
